High Risk Patient Tracking Sheet
A simple system for long term care dietitians to help you stay on top of your highest risk residents and never lose track of a follow-up.
Managing high risk residents in long term care can get overwhelming quickly. Between weight changes, wounds, poor intake, malnutrition risk, supplements, interventions, care plans, and follow-up dates, it’s easy for something to slip through the cracks.
This High Risk Patient Tracking Sheet gives you one organized place to track the residents who need closer nutrition monitoring.
For each resident, you can track their primary nutrition risk, malnutrition status, wound status and progress, weight changes and trends, average intake, diet and ONS, interventions, follow-up dates, consults, and whether the appropriate documentation has been completed.
When a resident’s nutrition risk is resolved, you can mark them off the list. If they remain high risk, simply enter their next follow-up date and continue tracking them.
The sheet also allows you to add the resident to your Monthly Follow-Up List, creating an easy system for making sure your high-risk residents continue to receive the follow-up they need.
Use it to:
• Keep your high-risk residents organized
• Track nutrition concerns and changes over time
• Document interventions and follow-up plans
• Keep important follow-up dates from being forgotten
• Monitor whether nutrition risks have been resolved
• Keep track of consults and charting
• Make your nutrition follow-up more consistent
Stop trying to remember everything in your head.
Give yourself a simple system for knowing who needs your attention, what you’ve done, what still needs to happen, and when you need to follow up.
Designed specifically for the busy long term care dietitian. A perfect long term care dietitian resource for you!
